Hospital Handover Before Opening: A Practical Readiness Checklist
10/21/20262 min read
Start with three separate readiness tests
A hospital is ready to open only when its handover obligations, statutory approvals and operational preparations have each been addressed. These are related but not interchangeable. Contractual handover requirements come from the construction contract and project specifications. Statutory approvals are permissions, inspections or certificates required by relevant authorities. Operational readiness confirms that staff, systems and procedures can safely support patient care.
Assign an accountable lead to each category and maintain a tracker showing the required evidence, responsible person, status and outstanding action. Do not treat a signed handover certificate as proof that every regulator has approved the facility or that clinical teams are ready to use it.
Confirm the contractual handover package
Check that the final approved drawings and specifications are complete, clearly identified and updated to reflect the installed facility. Include coordinated architectural, structural and building-services records, along with revisions, equipment locations and relevant schematics. Obtain operation and maintenance manuals for installed systems and equipment, including supplier contacts, operating limits, routine care instructions and recommended spare parts.
Collect test certificates, inspection records, commissioning reports and balancing results. Evidence should identify the system or asset tested, the method, results, date and responsible technician. Confirm that commissioning issues have been closed or recorded as unresolved defects with an owner and agreed completion date. Review warranties, guarantees, service agreements, asset registers and preventive maintenance schedules; check that asset identifiers and locations match what staff will find on site.
Check hospital infrastructure in detail
For medical gas pipeline systems (MGPS), verify required installation and performance test records, gas identity and outlet labelling, alarm operation, source equipment documentation and procedures for emergency isolation. Confirm that the responsible competent persons have reviewed the system and that staff know how to respond to alarms. The applicable tests and approvals depend on local standards and the project requirements.
For HVAC, review air balancing, temperature and humidity performance, pressure relationships, filtration and controls, especially in critical clinical areas. Confirm that electrical records cover protective devices, earthing, emergency power, transfer arrangements and testing of essential circuits. Check other building services—including fire detection and suppression, lifts, water systems, drainage, communications and security—for their relevant test results, operating instructions and maintenance arrangements.
Verify approvals and operational readiness
Build a separate register of statutory approvals and authority inspections required for the facility and its systems. Depending on jurisdiction, these may include building or occupancy approval, fire-safety clearance, elevator certification, environmental permissions and healthcare-specific licensing. Confirm which approvals must be in place before occupation or clinical use, who issues them and whether conditions remain outstanding. Contractual acceptance does not replace these permissions.
Finally, verify that operational teams have received role-appropriate training, attendance records and training materials. Walk through the facility with clinical, engineering, facilities and safety leads. Test access, alarms, escalation routes, emergency procedures and maintenance reporting. Confirm that essential supplies, service contacts and planned maintenance are in place. Record every remaining defect, assess its safety and operational impact, and agree controls and deadlines before opening. Where an issue could compromise patient safety or legal compliance, escalate it and do not assume it can be resolved after patients arrive.
